While genomics has contributed significantly to our understanding of many diseases, this new understanding has not necessarily translated into our ability to predict those at risk of disease, detect early disease onset, or better manage the course of patient treatment, ultimately leading to improved patient outcomes. What genomic research has taught us is that many diseases, such as cancer, are far more complicated than we appreciated, to the extent that our ability to “routinely” (and cheaply) sequence the human genome has not yielded the prognostic power we had expected. This complexity derives from the vast levels of molecular and cellular heterogeneity present in most cancers – heterogeneity that next generation sequencing alone does not capture. The genotype-to-phenotype relationship often exhibits poor correlation, making genotype alone an insufficient basis for patient diagnosis and treatment decisions. Given that the ultimate measure of the tumorigenicity of a given mutation is based on an assessment of the impact of that mutation on the function of the gene product (protein), either alone or in the context of a molecular network, the missing link between genotype and phenotype likely lies in the proteotype. This lecture will explore how new “multi-omics” capabilities are now positioned to improve the management of patients by providing precise and actionable molecular phenotypes based on proteogenomic analysis.
